- How to Request Your Medical Records
You have the right to inspect, review, or obtain a copy of your protected health information (medical records). To request your records:
- Submit a Written Request: Fill out our Medical Record Request Form, available at our front desk or online at texasmedicalinstitute.com
- Provide Required Details: Ensure your request includes your full legal name, date of birth, contact information, the specific dates or types of records requested, and whether you want them delivered via mail, secure email, or available for pickup.
