In accordance with Texas Health and Safety Code Section 181.105, please review the step-by-step instructions below regarding how to request your medical records, contact our licensing authorities, or file a consumer complaint.

  1. How to Request Your Medical Records

You have the right to inspect, review, or obtain a copy of your protected health information (medical records). To request your records:

  • Submit a Written Request: Fill out our Medical Record Request Form, available at our front desk or online at texasmedicalinstitute.com
  • Provide Required Details: Ensure your request includes your full legal name, date of birth, contact information, the specific dates or types of records requested, and whether you want them delivered via mail, secure email, or available for pickup.
  • Submit to:

  • Processing Time & Fees: We will fulfill your request within 15 business days. Reasonable fees for labor and supplies may apply as permitted by Texas law.
